The first step toward Illinois' Medicaid overhaul is the naming of a half-dozen care coordination networks to launch the transition. By 2015 the state wants to have half its Medicaid clients in coordinated care.
They coordination networks are:
Precedence Care Coordination
Together4Health
Community Care Alliance of Illinois
Macon County Care Coordination
Healthcare Consortium of Illinois
Be Well Partners in Health
"The health care and social service providers who participated in the Innovations Project have shown a tremendous willingness
to collaborate and test new models of delivering care to our most vulnerable populations" Illinois Department of Healthcare and Family Services director Julie Hamos said in a statement.
